Hereby the case of a middle aged female who presented with vaginal brownish discharge along with lower abdominal pain and primary infertility has been reported . Transvaginal ultrasonography showed right adnexal mass suggestive of hydrosalpinx. Intradermal tuberculin test was performed which atypically lead to formation of giant bulla of size 30×32 mm by 12 hours.
