The literature search was performed on databases Scopus, Web of Science, MedLine, The Cochrane Library, EMBASE, Global Health, and others. The article describes the most common causes of persistent elevation of hepatic transaminases in asymptomatic patients and the algorithm of diffe­rential diagnosis in hypertransaminasemia. The spectrum of diseases accompanied by the development of hepatic steatosis is presented, the issues of diagnosis of non-alcoholic liver disease and Wilson’s disease in children are covered. The attention is focused on the difficulties of interpreting clinical data and evaluating the laboratory and instrumental findings in this group of patients.
