Summary:Microphthalmos with cyst is a rare condition that challenges the surgeon in how to choose the best treatment option depending on the degree of ocular malformation, cystic component, and secondary orbital expansion. We present a case of a two-year-old female with Delleman syndrome, presenting with a blind eye, which had an extensive infero-lateral coloboma and a large cystic component inducing a significant enlargement of the orbital cavity with additional cosmetic detriment requiring surgical intervention.
