Summary:<p>Decision-making in the adaptation practice of the therapeutic conduct must be done taking into consideration a specific system of reference. This article deals with the topic in the Cuban social context, where the observance of the good clinical practices is compulsory. In this field, the knowledge and the application of ethical principles are essential. By means of comparisons the work criticizes the principalism and utilitarian theories, typical of the capitalist system. The aim is to clarify, in a precise way, the absence of the influence of those theories in the Cuban setting. The readers will have at hand some concepts and ways of thinking that are in force in contemporary ethical discussion.</p>
