Description
Summary:Introduction. Due to the fact that in recent years has been increased the prevalence of cancer, including among children, so it is intensified the debate on issues related problems of the presence of dental pathology in this category of patients. Carrying out the chemotherapy, except to the waited therapeutic effect, causing of significant harm, disturbing the homeostasis of the body, including the level of the mouth, which then has a negative impact, contributes to the intensity of caries occurrence and its complicated forms. The dental toxycation of cytostatics increases in accordance with the number of courses of chemotherapy. The study of features of homeostasis system on the back ground of cancer pathology course recently received much attention. The research is carried out in order to improve treatment and developing a system of preventive measures for the period of receiving chemotherapy. The results of scientific elaborations of recent years allow to assume that oral fluid is a part of a system of biological fluids, which is sensitive to changes in its components in various pathological conditions. It has a powerful enough homeostatic capacity and is able to perform several important functions primarily mineralizing one, and leading role in the implementation of which belong to calcium and phosphorus. The completed mineralization of tooth tissues is possible in normal functional activity of salivary glands, with secret of which calcium and coupled with it phosphorus by metabolism came in enough number. General causes of the reduction in calcium, including in the oral fluid are a disturbance of absorption in the intestine and its reabsorption in the kidneys by a number of pathological conditions. Taken together, these factors can be realized and on the ground of malignant process and the toxic effects of chemotherapy drugs. However, information about the content of calcium, phosphorus and level mineralizing potential of oral fluid in children with malignant tumors of soft tissues under the influence of cytostatic treatment is limited, hence the topicality of this study. The aim of our study was to determine the effect of gel preparations "Zoleh" and "Slyurem" on the speed and level of salivation of mineralizing potential of oral fluid in children with malignant tumors of soft tissues in the dynamics of chemotherapy. Materials and methods of researches. For the study it was selected a group of 28 children of the age from 7 to 15 years with malignant tumors of soft tissues of different anatomical location of morphologically verified and sensitive to chemotherapy: The control group consisted of 25 somatically healthy children of the same age. Children who received chemotherapy, depending on the amount of medical dental measures were divided into two groups. The first, consisting of 13 patients treated according to the protocol of medical care given patients, and a traditional complex on oral care. The second group of 15 people in addition to the basic treatment as replacement therapy intended similar to gel preparations "Zoleh" and "Slyurem". Conclusion. In children with malignant tumors of soft tissues of different anatomical location it is traced progressive decreasing of mineralizing potential of the oral fluid at receiving courses of chemotherapy at all stages of monitoring. In particular, on the back ground decreasing of speed of salivation it is changed the contents of Ca and P, resulting in broken their relationship, which is accompanied by a decrease in calcium-phosphorus coefficient. The introduction to the integrated treatment of these patients by the similar gel forms "Zoleh" and "Slyurem" can partially offset the imbalance between the processes of re- and demineralization that substantiates the expediency of their use during the period polychemotherapeutic treatment. Possibility of further researches. Timely determination of changes in the components of mineralizing potential of the oral fluid in children with malignant tumors of soft tissues that occurs directly under the influence of combination chemotherapy can be recommend the introduction of additional preventive measures aimed at their correction. It prevents the occurrence and progression of dental pathology and needs further study of their effectiveness in the nearest and long terms.
