Summary:Background: Enabling improved health outcomes and experiences of care for people living with chronic and complex conditions whilst delivering better value care requires a complex co-designed digital approach. The NSW Agency for Clinical Innovation (ACI) has supported, implemented and evaluated an initial co-designed proof of concept program for routine collection and use of Patient Reported Measures (PRMs). The PRM program was implemented as a key enabler of the NSW Integrated Care strategy across care settings in real-time to support improved health outcomes and experiences of care. PRMs are surveys that measure a persons perception of their care experiences and the impact of conditions and treatment on their health and quality of life. PRMs enable patients to be an active participant in their care, identifying what really matters to them. The evaluation of the program has led to a robust co-designed PRM Program and procurement of a digital platform capable of delivering PRM results in real time to healthcare providers to inform clinical care, consultations and improve shared decision making processes. Furthermore the platform enables patients to create their own portal to view results, self-management information and track their own progress over time. The new PRMs IT solution will be rolled out in a phased approach and integrated into electronic Medical Records (eMR) and General Practice (GP) software. Aims and Objectives: This interactive workshop will take participants through the journey of embedding PRMs into routine care. Tools and resources will be shared that increase consumer, clinician and manager capability and support the meaningful use of experiences and outcomes. Participants will participate in facilitated discussions and small group activities about practical strategies for implementing PRMs into ‘business as usual’ and considerations for scalability and sustainability, including barriers and enablers for co-designing and implementing a digital platform across care settings for real-time feedback, including an IT demonstration. Key findings from the PRMs evaluation will be shared along with lessons learned from our experience of implementing PRMs within complex healthcare systems.
