To this end, FSAQ was administered primarily to 153 subjects after preparation. Afterwards, two groups of clinical and non-clinical subjects were studied. The non-clinical subjects comprised of 530 Gilan University students (265= males 264= females and one non-identified), while 39 subjects with anxiety disorders were studied as clinical subjects. In addition to providing the descriptive analysis related to gender and faculty, statistical methods such as test-retest, split-half and Cronbach Alfa, was used to determine the reliability of the questionnaire. The results confirmed the reliability of questionnaire and its factors. Further more, the “Cattle Anxiety Scale”, “Beck Anxiety Questionnaire”, “Spilberger State-Trait Anxiety Inventory”, “MMPI (A)”, and “General Health Questionnaire” (GHQ-28) were used to approve of the validity of the questionnaire via the correlation between the questionnaire and other tests was determined. The results showed the convergent validity of the questionnaire.
