Summary:Background: Degenerative lumbosacral spine disease is common among the middle aged and elderly patients with a spectrum of neurological problems. Priapism is a known but rare presentation of the degenerative lumbosacral spine disease. Methods: Two patients who presented with socially embarrassing priapism associated with low back pain and neurogenic claudication were evaluated with lumbosacral spine MRI. Results: The lumbosacral spine MRI of both patients showed lumbar spinal canal stenosis with posterior intervertebral disc prolapse and cauda equine compression. The patients had lumbar decompressive laminectomies with complete resolution of their symptoms. Conclusion: Priapism is a rare but known presentation of degenerative lumbosacral spine disease. Patient with new onset priapism should be evaluated with lumbosacral spine MRI most especially if there is background history of recurrent back pain associated with neurogenic claudication. Decompressive laminectomies is an efficient modality of treatment.
