Summary:Primary carcinosarcoma of the ovary is a rare, challenging malignancy of the female genital tract. Preoperative diagnosis of this tumor is seldom made as it mimics epithelial ovarian tumors. We report a case of 45 years multiparous lady who underwent staging laparotomy to disclose smooth surface left ovarian cystic mass with minimal solid areas. Histopathologically a biphasic tumor with malignant epithelial and mesenchymal component was seen. Explicit diagnosis of primary ovarian carcinosarcoma was established using immunohistochemical marker study. Primary mixed Mullerian tumor of the ovary has to be considered in the differential diagnosis of predominantly cystic ovarian lesion with minimal solid areas. Regular followup and close monitoring of the patient is required to understand the behavior of this exceptional tumor.
