Material and methods. There were operated 18 patients (18 eyes) at the age from 60 to 77 years with IMH of 3-4 stages upon J.D.M. Gass classification (1988). Endovitreal intervention consisted in carrying out 3-port 25G vitrectomy with excision of an internal limiting membrane (ILM) and rapprochement of gap’s edges by means of a passive aspiration. Further the tamponade of an eye cavity was carried out with light silicone. Results. As a result of surgical treatment a complete closing of macular holes was achieved in 94.4% of cases. Visual acuity after silicone removal remained without changes in 5 cases, improvement was obtained in 13 cases (on the average by 0.1). Conclusions. Use of a silicone tamponade in a combination with a technique of rapprochement of gap’s edges by means of a passive aspiration at surgical treatment of IMH of big diameters leads to gap’s closing in many cases, to recovery of fovea anatomy, and to visual acuity rising in the majority of patients. http://www.eyepress.ru/article.aspx?11559
