Summary:Diagnosis of ALS is based on clinical examination and electromyographic tests which are used to confirm the clinical suspicion and to exclude alternatives. Atypical presentations may delay the diagnostic and subject the patient to unnecessary tests and procedures. We discuss here three cases with atypical features: head dropping, highly focal weakness and amyotrophy and the last case with surgery for lumbar stenosis.
