The subject of the present study was one female late preterm neonate with very low birth weight (1.48 Kg). She needed mechanical ventilation because her low oxygen saturation. Gastric perforation and pneumoperitoneum occurred secondary to mechanic ventilation. She had an abdominal paracentesis and an emergency laparotomy. The gastric perforation was repaired, banding of the fistula was applied and gastrotomy tube was inserted.
