METHODS TO IMPROVE FIXATION OF THE PIN CONSTRUCTIONS

Summary: | Introduction. The saving of the functional wholeness of the tooth alignments is a priority of prosthetic dentistry. Modern technologies allow using for prosthetic teeth with a crown completely destroyed in such a clinical situation, the preference is given to manufacturing of the pin constructions. However, the main their drawback is the lack of reliable fixation in the root canal, which leads to the development of complications over time. We compared the degree of fixing one root pin constructions depending on the fixing of cement and method of processing the root’s part of the pin structures. Improving the quality of fixation of the pin constructions improves their functioning and reduces the probability of complications after cementation.
Objective. The aim of our study is to improve the fixation of metallic root pin structures through a rational choice of cements for fixation and the method of processing the root’s part of the pin structures.
Methods. For research we were used 90 of the canines and incisors of the upper and lower human jaws. Extracted teeth were disinfection in the 2% solution of Korsolex for 1 hour and then it was stored in physiological solution. The crowns of the teeth were cut with a diamond bur. The root canals were drilled to a depth of 12 mm. Next we were made the single-root pin constructions. The root pin constructions were cemented on the zincphosphate cement "Adhesor" ("SpofaDental"), glass ionomer cement "Meron" ("VOCO") and double-curing composite cement "Jen-Dua Cem" ("JenDental"). Then pin structures were fixed in a block of plastic. The strength of fixation was measured on a testing machine FU-10000ЕZ at the Dnipropetrovsk Center of Standardization and Metrology.
Results. During the research it was found that the strongest fixation characteristics of the zinc-phosphate cement "Adhesor". The amount of force is at break, when pulling single-root pin construction occur without preprocessing, for this material has averaged 0,93±0,14 MPa, which exceeds the value for glass ionomer cement "Meron" of 1.16 times and double-curing composite cement "Jen-Dua Cem" to 1.52 times. Comparison of pin fixation constructions without pre-processing and with pre-sandblasting of its root parts was showed an increase of strength at break for the zinc-phosphate cement "Adhesor" and glass ionomer cement "Meron" to 1.23 and to 1.14 times, respectively. For pin constructions with preliminary mechanical processing of the root parts was demonstrated an increase in strength at break for glass ionomer cement "Meron" in 1.35 times compared with the first group and in 1.17 times compared with the second group. We didn't find any significant difference in the degree of fixation among the pin constructions, which were fixated on the double-curing composite cement "JenDua Cem".
Conclusions. The preliminary mechanical processing of the root’s part of the pin constructions before cementing on the "Adhesor" significantly improves the strength of fixation. However, the degree of fixation of the pin constructions on the "Adhesor" when pre-sandblasting processing and applying notches almost equivalently are determined. Therefore, we can conclude that to improve the fixation characteristics of the single-root pin constructions quite pre-sandblasting processing. The preliminary mechanical processing of the root’s part of the pin constructions before cementing on the "Meron" greatest extent increases the degree of fixation of the pin constructions. Double-curing composite cement "Jen-Dua Cem" has the lowest amount of force at break, consequently it is not recommended for fixation of the pin constructions. |
